Case Management
Our case managers provide access and linkage to appropriate services for HIV-positive individuals, encouraging personal empowerment, and honoring the uniqueness of every individual regardless of race, ethnicity, sexual orientation, religion, gender and differences of ability. Case managers provide the following:
Identification of needs
Knowledge of availability of services
Connection with state programs
Help in reducing barriers to services
Client advocacy
Informational mailings regarding new treatment/services
